Patient Notice – FMLA Paperwork Administrative Fee

At Therapeutic Village, PLLC, we are committed to assisting our patients with documentation needed for employment or medical leave purposes. Please note that the completion of Family and Medical Leave Act (FMLA) forms requires additional administrative time outside of your clinical appointment.

• The standard fee for FMLA paperwork is $100.
• Fees may be higher depending on the complexity of the forms or if additional documentation is requested.
• Payment is due at the time the forms are submitted for completion.

We appreciate your understanding, as these administrative services are not covered by insurance. Our goal is to provide thorough and accurate support while ensuring that the process is clear and convenient for you.
